About the Event 许多临终病患表示希望在家中度过最后时光。但对于需要生命维持系统的临终病患而言，从加护病房转送回家存在一定的难度及风险。 IAmGoingHome是由黄廷方综合医院推出的一项多学科临终出院项目。本次讲座将分享如何通过看护者培训、症状与疼痛控制、设备配置等措施，让患者安全且有尊严地返回家中，在熟悉的环境与亲人的陪伴下走完人生最后一程。 Speakers’ Profiles Leong Shiaw Yi (Ms) Assistant Nurse Clinician Intensive Care Unit Ng Teng Fong General Hospital 梁孝仪现任黄廷方综合医院加护病房（ICU）助理临床护理导师。她持有护理学学士学位，自2013年起成为注册护士，拥有丰富的重症护理经验。 秉持全人及以患者为中心的护理理念，梁孝仪特别关注重症医疗环境中的安宁医疗与临终护理。她于2023年加入ICU安宁医疗团队，并积极参与IAmGoingHome计划的建立与推行。为进一步提升相关专业能力，她接受了安宁医疗培训，并持续推动将安宁医疗理念融入重症护理实践。 梁孝仪致力于提升临终照护质量，与加护病房医生及跨专业团队紧密合作，为生命受限疾病患者提供更具关怀及以患者为中心的照护服务。她也积极参与加护病房护士培训与指导工作，涵盖关怀出院规划及临终照护实践，协助护理人员为患者及家属提供有尊严且全面的支持。 Lim Huey Ying (Dr) Principal Resident Physician Intensive Care Medicine Ng Teng Fong General Hospital 林潓音医生现任职于黄延方综合医院加护医疗科首席住院医生。同时也是加护病房 （ICU） 安宁医疗团队的核心成员。 林医生持有安宁疗护医学研究生文凭，专注于重症医疗与生命末期照护，并积极推动安宁疗护理念融入重症监护。 她也是 IAmGoingHome计划的发起人之一，致力于协助合适的重症临终病患实现回家的心愿，让他们在人生最后的时光回到熟悉的家中，在挚爱亲人的陪伴下，安宁而有尊严地走完生命最后一程。 This programme is suitable for seniors. Registration and Attendance Registration is required for this programme. Please register one ticket per person. Attendance will be taken using the NLB Mobile app. Please download the app (https://go.gov.sg/nlbmobile-attendance) or update it to the latest version.
